Care Pathways - Steering Groups
The first meeting brought together 24 key stakeholders who helped to shortlist from 13 to 6 conditions for further consideration for Care Pathway mapping.
Steering Group 2 - 15th June 2015
Following an extensive round of engagement, the 2nd meeting resulted in the 6 shortlisted conditions being shortened to 3; Acute Asthma (Paediatric), Diabetes (Foot) and Heart Failure.
This was subsequently reduced to 2 having discovered that a seperate Regional Group had been tasked with developing a Care Pathway for Diabetes.
Steering Group 3 - 10th August 2015
This meeting considered the potential impact of focussing on developing/refining Care Pathways for Acute Asthma (Paediatric) and Heart Failure. This was informed by a significant ammount of data analysis carried out by the HSCB.
The group also discussed the best approach to pathway design, the most relevant parts of the pathway to focus on (diagnosis, referral, discharge) and agreed to establish multi-disciplinary groups to take forward the work.

Steering Group 4 - 23rd October 2015
This meeting updated on progress made through the MDG workshops for Asthma and considered implementation of the pathway. Approach for Heart Failure MDG workshops was also agreed.
Steering Group 5 - 11th December 2015
This meeting updated on progress made through the MDG workshops for both workstreams. Implementation planning for Asthma was discussed and the Heart Failure pathway continues to be developed.
